MISSION
The mission of Lifetime Support, Inc.
is to promote a life of dignity and opportunity for persons with
developmental disabilities in need of advocacy and guardianship
services and to ensure that their needs, strenghts,
talents and desires are affirmed, defended and respected. Guided by
person-focused values and least restrictive tenets, Lifetime Support
is committed to maximizing the autonomy and self-determination of
persons requiring assistance, while protecting them and fostering
quality in their lives.

For the past few years,
families have expressed the need for an organization that could take
care of their children once they were no longer able to do so. An
organization that could provide guardianship, financial/trust planning,
payeeship and advocacy for the family member.
The Arc of Union County developed a separate non-profit corporation, Lifetime
Support, Inc., to meet this need.
Lifetime Support is designed to offer parents and other family members
an opportunity to receive assistance in the following areas:

As guardian or
co-guardian of an individual, Lifetime Support will assure that the
individual's interests will be protected during their lifetime.
Following the family wishes and letter of intent information, the
guardian will act to protect these interests and all human rights.
|
|

|
|

|
|
This trust permits the
pooling of resources from several families to benefit each individual
family member. The Community Trust provides an efficient management of
a small legacy or trust fund to ensure ongoing care and monitoring of
the individual.

|
As payee on Social
Security Administration checks and other payments, Lifetime Support
reviews and pays all bills submitted for the individual. The bank
account is reconciled monthly and a financial statement is prepared.

|

|
|
A formal Care Plan is
developed once the application process is complete and guardianship is
obtained. The Care Plan identifies services and supports to be provided
by Lifetime Support which most appropriately meet the individual's
needs while at the same time maximizing autonomy and
self-determination. Development entails an interview with the
individual and the family to determine wishes, review of the letter of
intent, meetings with the Interdisciplinary Team, review of any burial
plans and meetings with staff from the individual's day and residential
programs. Long and short term goals are developed.

Lifetime Support advocates
for the individual at meetings with family members, Interdisciplinary
Teams, agencies and other service providers.
|
|

|
|

|
|
Monthly visits will be
made to any individual for whom Lifetime Support provides guardianship
for the purpose of determining any needs, reviewing quality care,
employment/day program situations and the health and happiness of the
individual. Each visit is fully documented.
